How to Verify a Vendor Really Books Into Your EHR Before You Sign

· , Founder & CEO

To verify a vendor really books into your EHR, have it book, move and cancel a test patient in your live schedule before you sign, then check the EHR, not the vendor's dashboard.

"Integrates with your EHR" can mean anything from a nightly export to real-time booking. Below: six acceptance tests, four integration levels, and contract language.

How do I verify a patient communication vendor actually integrates with our EMR before signing?

Run six tests on a test patient in your real schedule, with your staff watching the EHR. If any result only shows up in the vendor's own app, the integration is one-way or delayed.

Test What to do Pass if
Live slot lookup Block a provider's 2 pm in the EHR, then ask the vendor for that slot 2 pm is never offered
Rule-compliant booking Request a new-patient visit in an existing-patient-only slot Vendor refuses or offers a valid slot
Write-back Book a follow-up through the vendor It appears in the EHR schedule within a minute, right visit type and provider
Patient match Call as an existing patient, then as a new one Existing chart is found; no duplicate chart is created
Cancellation Cancel through the vendor, then cancel a different visit in the EHR Both slots show as free on both sides
Outside bookings Book a slot through your portal or a third-party site Vendor treats it as taken

A demo on the vendor's sandbox proves the software works, not that it works on your build of the EHR.

Why doesn't online scheduling actually book or confirm appointments?

Many "book online" buttons are request forms that staff confirm later by phone. Others book live, but only for a few simple visit types, because practice rules are hard to fit in a web widget.

Adoption reflects that. In a November 2024 MGMA Stat poll, 73% of medical group leaders said a quarter or fewer of their patients self-schedule digitally, and only 11% said a majority do.

Booking path Confirmed on the spot? Main failure
Online request form No Patient never hears back, or the slot is gone
Live online self-scheduling Yes, for opened visit types Most visit types stay phone-only
Third-party booking site Depends on the connection Lands in a separate inbox and gets re-keyed
Staff on the phone Yes Holds and voicemail when lines are busy
AI phone agent with EHR write-back Yes Only as good as the rules it is given

Most of the gap is visit types that were never opened to self-scheduling, not patients who refuse to use it.

What does "integrates with our EHR" usually mean?

It usually means one of four levels, and only the last two book appointments. Certified EHR APIs alone don't guarantee scheduling access.

The federal standard for certified EHR APIs, ONC's §170.315(g)(10), requires "read" services and specifically excludes "write" capabilities. Scheduling sits in separate FHIR resources (Appointment, Slot and Schedule) and operations such as the Argonaut Scheduling $book, which each EHR vendor may or may not expose, and which each practice must switch on.

Integration level What the vendor can do What breaks
File export (nightly CSV) Send reminders from yesterday's schedule Reminders for visits cancelled today
Read-only API See appointments and patient details Can't book; staff re-key requests
Booking request queue Drop requests for staff to approve Patient leaves without a confirmed time
Live read and write Search open slots, book, move and cancel in the EHR Needs rules configured per provider

Ask which level, for which actions, on your EHR version. "Yes, we integrate" answers none of these.

How can I get my staff off phone scheduling while still respecting each doctor's scheduling rules?

Write the rules down once and let the booking agent apply them on every call: which visit types each provider takes, which providers are interchangeable, and what counts as urgent.

YouSquared's AI Receptionist books, reschedules and cancels directly in the practice's EHR schedule, recognizes existing patients, and follows provider availability from the EHR calendar: block time in the EHR and the AI stops offering it. It connects to a dozen EHRs, including Epic, athenahealth, eClinicalWorks, Practice Fusion, Nextech, ModMed, WebPT and OncoEMR.

Rule the practice sets Example What the caller experiences
Visit types per provider Dr. A sees new patients Tuesdays only Only valid slots are offered
Interchangeable providers Any of three GI doctors for a colonoscopy consult A sooner slot with a similar doctor
What counts as urgent Post-procedure bleeding, chest pain Flagged and transferred to on-call with a summary
Who gets which transfers Billing to the billing office, refills to nurses Warm transfer to the right person

From our logs, a slot search takes about 1 second; human schedulers in our mystery-shopper calls took a median 88 seconds.

Results: 69% of slot searches end in a booking

In YouSquared call data (2026), when the AI searched the EHR for slots, 69% of those calls ended with a booking or reschedule at two primary care practices in New York, and 54% at a 12-location gastroenterology group in New York.

Practice Calls resolved without staff Slot searches ending in a booking Cancellations arriving after hours
Primary care, two practices (over 8,000 business calls) 52% 69% Not measured
Gastroenterology, 12 locations 75% 54% 32%

Primary care transfers more by design: there the AI handles overflow and after hours, and hands back to staff during open hours.

At the GI group, 32% of cancellations came after hours. Written straight into the EHR, those slots are open by morning, not after someone clears voicemail. And 42% of patients offered a sooner similar doctor took it (load rebalancing).

What contract language should we ask for?

Put the acceptance tests in the contract, with a right to exit if they fail on your live EHR. Pushback on that clause tells you which integration level you're buying.

Clause Sample wording
Integration level "Vendor reads open slots and writes, reschedules and cancels appointments in Customer's [EHR] in real time."
Acceptance "Go-live requires passing the attached acceptance tests on Customer's production schedule. Customer may terminate without fee if not passed within 30 days."
Rules "Vendor books only visit types, providers and locations permitted by Customer's written scheduling rules."
Fees "All EHR interface, API and per-transaction fees are listed in Schedule A. None are added later."

Ask who pays the EHR vendor's interface fee: it often sits outside both quotes.

How to set it up

Send us your provider list, visit types and scheduling rules. We connect your EHR, configure who can be booked for what, and you run the acceptance tests above on a test patient before going live.

Key terms

  • Write-back. The vendor creates or changes the appointment in the EHR itself, not in its own app.
  • FHIR. HL7's standard for exchanging health data through APIs. Appointment, Slot and Schedule are its scheduling resources.
  • §170.315(g)(10). ONC's certification criterion for standardized EHR APIs, limited to read access.

FAQ

Which AI products integrate with eClinicalWorks for a medical practice? Ask each vendor to pass the six tests above on your eClinicalWorks instance. YouSquared books, reschedules and cancels directly in eClinicalWorks.

Why do appointments booked through third-party portals like ZocDoc get lost by the office? When the portal sends bookings to a separate inbox instead of writing to the EHR, staff must re-key them, and some are missed. Ask whether bookings write straight into the schedule.

How do we keep the schedule full when patients cancel, with Allscripts integration? Cancellations need to write back to the EHR in real time so the slot is offered again right away. YouSquared can integrate an EHR outside its current list on demand.

Is it secure for a medical practice? Yes. YouSquared is HIPAA compliant, SOC 2 audited, and signs a business associate agreement (BAA). See Security.
